Cameron Mathison’s version of Drew on General Hospital was introduced on the August 16, 2021 episode. He was revealed to have been held captive and brainwashed by Victor Cassadine.
Back Home
Once back in Port Charles, Drew reconnected with his daughter Scout (Cosette Abinante) and other family members. However, he dealt with guilt over Jason’s (Steve Burton) presumed death (he was the one rescued), survivor’s remorse, and struggling to bond with Scout, who hardly remembered him.
Drew and Carly (Laura Wright) began getting closer throughout 2022 — they shared kisses at the Quartermaine picnic and at Jacksonville Beach, eventually embarking on a romantic relationship. Tensions arose due to their insider-trading legal issues. Drew and Carly were caught in an insider trading scandal, which he took full responsibility for to protect Carly. Initially expecting a light sentence, Drew was instead sentenced to three years at Pentonville Penitentiary in mid-2023. He endured threats and violence inside prison, including a severe beating, but was unexpectedly released early after Carly and Diane (Carolyn Hennesy) successfully challenged the judge’s decision.

Upon his return, Drew exhibited marked changes—appearing more hardened and aggressive. He became politically ambitious, especially with his business pursuits and securing a congressional run. His darker, less altruistic side shifted him into romantic plotlines with Nina (Cynthia Watros) and later, Nina’s daughter, Willow (Katelyn MacMullen). In 2024–2025, Drew entered a controversial affair with Willow while simultaneously being involved with her mother. Once his feelings for Willow became more intense they shared two kisses, then finally succumbed to their passion, having sex on the floor of the Quartermaine nursery.
In the last year, Drew has blown up his friendship with Curtis (Donnell Turner), Portia (Brook Kerr), and battled with his family, The Quartermaines. His involvement with Willow, just lost her custody of her two children, Wiley (Viron Weaver) and Amelia when Michael (Rory Gibson) was awarded full custody.
Not only is this massive shift in character unlike the original incarnation of Drew, played by the late Billy Miller, it is a total 180 from when Drew returned in the form of Mathison. He has betrayed literally everyone, from Nina, to Willow, to the Quartermaines and is showing very little remorse, even going so far as to tell Wiley that Michael did not come back to Port Charles to be with him, he returned to be with his new daughter, Daisy, with Sasha (Sofia Mattson).

Fans are pretty outraged, and rightfully so. There are many theories floating around – Drew had some sort of brain damage after his severe beating in Pentonville, that he is actually a returned from the dead Shiloh (Coby Ryan McLaughlin) or that he just is a complete jerk.
Here are my issues with the options above – First, GH just did a character has a medical diagnosis that explains their behavior, think Heather Webber (Alley Mills).
As far as the resurrected Shiloh potential, that seems far fetched, even for a soap. Shiloh was shot at by Jason during a struggle—but his bulletproof vest swallowed the shot, allowing him to continue his attack. As Shiloh prepared to kill Jason on the ship’s deck, Sam (Kelly Monaco) intervened, firing a flare gun directly into Shiloh’s back. The impact sent him staggering over the railing and plunging into the sea. Shiloh’s body was later pulled from the water and brought to General Hospital in a body bag. There, Willow positively identified him, confirming his death. Shiloh did control Willow and this could be his ultimate revenge – costing her the children. Could Victor have taken Shiloh’s body and resurrected him, giving him Drew’s face and swapping out Shiloh for Drew?
My thoughts are it is just a change in character. But that seems too easy and not very soapy, which may be the point. Maybe it is leading up to a “Who Killed Drew?” storyline. At this point a good portion of the citizens of Port Charles could very well be a suspect.
