Rick was put to the test by Negan when he ordered him to cut off his own son’s arm in Season 7 of The Walking Dead, but he didn’t end up doing it.
Back “home”, Carl asks Rick if he actually could have done it and Rick has to admit he doesn’t know. Carl tells his father they could be in that situation again, but Michonne puts a stop to that talk saying next time they’ll be ready.
